Pediatric Home Care RN Position

Responsibilities:

Patient Assessment: Conduct thorough assessments of pediatric patients to determine their health status, needs, and developmental milestones. Monitor vital signs, growth parameters, and overall progress regularly.

Care Planning and Implementation: Develop individualized care plans in collaboration with the interdisciplinary team and patients' families. Implement nursing interventions, administer medications, and perform treatments according to established protocols.

Family Education and Support: Educate patients' families about pediatric health conditions, treatment plans, and home care procedures. Provide guidance and support to help families manage their child's health effectively and promote a safe and nurturing environment.

Interdisciplinary Collaboration: Communicate effectively with physicians, nurse practitioners, therapists, and other healthcare professionals involved in the care of pediatric patients. Collaborate with interdisciplinary team members to coordinate care and ensure continuity of services.

Documentation and Reporting: Maintain accurate and timely documentation of all patient care activities, assessments, interventions, and outcomes. Complete required documentation in electronic medical records and communicate significant findings to appropriate team members.

Qualifications:

• RN License: Valid Registered Nurse (RN) license in good standing.
• Pediatric Experience: Minimum of 1 year of clinical nursing experience, with specific experience in pediatric nursing preferred.
• Clinical Skills: Strong assessment, critical thinking, and clinical skills specific to pediatric nursing. Ability to respond effectively to pediatric emergencies and provide age-appropriate care.
• Communication Skills: Excellent communication and interpersonal skills. Ability to communicate effectively with pediatric patients, their families, and interdisciplinary team members.
• Flexibility and Compassion: Ability to adapt to the unique needs of pediatric patients and their families. Demonstrated compassion, empathy, and patience in caring for pediatric populations.
