Certified Nurse RN/LPN

Responsibilities:

Patient Care: Provide direct nursing care to residents according to established care plans and protocols. Administer medications, perform treatments, and monitor patient vital signs. Ensure the physical and emotional well-being of residents under your care.

Assessment and Documentation: Conduct thorough assessments of residents' health status and document findings accurately and timely in electronic medical records. Communicate significant changes in residents' conditions to appropriate healthcare team members.

Collaboration and Communication: Collaborate with interdisciplinary team members to coordinate resident care effectively. Communicate with physicians, nurse practitioners, therapists, and other healthcare professionals to address resident needs and concerns.

Family Support: Provide support and education to residents' families regarding resident care, health conditions, and treatment plans. Maintain open communication with families and address their questions and concerns promptly and compassionately.

Quality Improvement: Participate in quality improvement initiatives and adherence to regulatory requirements. Identify areas for improvement in nursing care delivery and contribute to the development and implementation of best practices to enhance resident outcomes.

Qualifications:

• RN/LPN License: Valid Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in good standing.
• Clinical Experience: Minimum of 1 year of clinical nursing experience preferred.
• Clinical Skills: Strong assessment, critical thinking, and clinical skills. Ability to provide compassionate and competent nursing care to residents.
• Communication Skills: Excellent communication and interpersonal skills. Ability to communicate effectively with residents, families, and interdisciplinary team members.
• Compassion and Empathy: Demonstrated compassion, empathy, and patience in caring for elderly and/or disabled individuals.
