Centers Healthcare, a premier healthcare provider, is seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our Assisted Living Program team. This role is pivotal in delivering exceptional care and support to our residents, promoting their health, well-being, and quality of life.

Key Responsibilities of LPN:

• Nursing Care: Administer medications, treatments, and interventions in accordance with physician orders and established protocols to ensure optimal resident health outcomes.
• Health Monitoring: Conduct regular assessments of residents' health status, document observations accurately, and communicate any changes or concerns to the interdisciplinary healthcare team.
• ADL Assistance: Provide hands-on assistance with activities of daily living (ADLs) such as bathing, dressing, grooming, and mobility, fostering independence and dignity for residents.
• Documentation: Maintain thorough and precise documentation of all care provided, including medication administration records, vital signs, and progress notes, ensuring compliance with regulatory standards.
• Collaboration: Collaborate closely with Registered Nurses (RNs), Certified Nursing Assistants (CNAs), and other healthcare professionals to develop and implement individualized care plans tailored to meet the unique needs of each resident.
• Emotional Support: Offer compassionate companionship, active listening, and emotional support to residents and their families, creating a supportive and nurturing environment within the community.

Qualifications for LPN:

• Licensure: Current licensure as a Licensed Practical Nurse (LPN) in good standing with the state board.
• Experience: Previous experience in assisted living or long-term care settings preferred, with a strong understanding of geriatric nursing principles and practices.
• Communication Skills: Excellent verbal and written communication skills, with the ability to effectively communicate with residents, families, and team members.
• Team Player: A collaborative and cooperative mindset, with the ability to work harmoniously within a multidisciplinary team environment.
• Compassion: Demonstrated empathy, compassion, and respect for the dignity and autonomy of each resident, coupled with a genuine passion for elder care.
