Overview: We are seeking a dedicated and compassionate Certified Home Care RN to join our team. As a Certified Home Care RN, you will play a crucial role in providing high-quality, personalized care to our patients in the comfort of their own homes. You will work collaboratively with our interdisciplinary team to ensure the best possible outcomes for our patients.

Home Care RN Responsibilities:

• Conduct comprehensive assessments of patients' health status and develop individualized care plans. Provide skilled nursing care, including wound care, medication management, and infusion therapy, by physician orders.
• Educate patients and their families on disease management, medication administration, and safety precautions.
• Monitor patients' progress and response to treatment, and make necessary adjustments to care plans.
• Coordinate with other healthcare professionals, such as physical therapists, occupational therapists, and social workers, to ensure holistic care. Document all patient care activities accurately and in a timely manner.
• Collaborate with community resources to facilitate continuity of care and support services for patients.
• Participate in quality improvement initiatives to enhance patient outcomes and satisfaction.
• Adhere to all regulatory requirements and standards of practice governing home care services. Maintain confidentiality and respect patients' rights and dignity at all times. Home Care RN

Qualifications for Certified Home Care RN:

• Valid Registered Nurse (RN) license in the state.
• Certification in home care nursing (CHPN) preferred. Minimum of 2 years of experience in home care or a related field.
• Strong clinical assessment and critical thinking skills. Excellent communication and interpersonal skills.
• Ability to work independently and as part of a team.
